Exception handling

I've made some progress in transferring Christophe's exception handling
specification to the Level II specification in the ABI.  Most of what I
have is taken directly from Christophe, but there are editorial changes,
some organizational changes, and extensive omissions of material that
are implementation (Level III) rather than required interface (Level II).

Please take a look, especially if you're involved in exception handler
implementation.  As with the rest of the document, the new material is
in red to facilitate its identification, and I'll convert it to black
after each meeting has given people a chance to note problems in the
new material.  With luck, Level II will be complete within a couple of
meetings.  I think this is the last major undone piece...
